High Five Program

In January 2011, The Urology Team instituted the High Five Program allowing patients the opportunity to give their provider or Urology Team staff members a High five for their exceptional service. Since its inception, our employees have received over 8,000 High Fives! Providers have received just as many!

After your visit, you will receive an automatic email asking you to rate your patient experience. You will also have the opportunity to give High Fives to providers and staff. It only takes a few minutes and we truly appreciate your feedback. Thank you!

Patient Survey

After visiting the Urology Team, patients will receive an email requesting feedback about their appointment. This very short survey will give us an immediate assessment of our provider services. After responding to a short list of questions, the survey will be submitted via email.
