Medical History & Physical instructions
Thank you for choosing Urology Team to meet your urological needs. A history and physical is required paperwork for your new visit and may also be required for yearly visits. Please choose if you would like to complete the paperwork electronically or print and fax to the office. Unfortunately, paperwork that is not submitted to the office at least 72 hours before scheduled appointment may require rescheduling of your appointment.

DROP BY OFFICE BEFORE APPOINTMENT OR RETURN IN STAMPED ENVELOPE:After printing, completing and mailing, please call the office 3 days prior to your visit to verify that your paperwork has been received. If you mail your paperwork and it is not received in the office on a timely basis then it will be necessary to reschedule your appointment. BRING DAY OF THE APPOINTMENT:As a last resort, bring your paperwork 45 minutes before your appointment. Unfortuantely, your visit will be delayed and may require that we reschedule your appointment. Please make every effort to get the paperwork to office before you appointment. |
